If you wish to submit a form to Cheshire West and Chester Adult Social Care you will need to create a portal account.

How to Register for a Portal Account

To be able to register for a Portal Account you will need to have a valid email address.
Go to My Account and select Register for a new account
Once you have entered your registration details you will be sent a verification code to the email address that you have provided.
You will need to enter the Verification code in order to finalise the registration.
You will have 10 minutes to enter the code after which time it will EXPIRE and you will need to start the Registration process again.
