The Prevention and Wellbeing, Adult Mental Health Team is a bespoke Adult Social Care care provider in Cheshire West and Chester.    We work alongside a wide range of consultants, clinical experts and specialised agencies providing rehabilitation, treatment programmes, care planning, assessment, and long-term focus on recovery and overall wellbeing.   This portal form is for Cheshire and Wirral Partnership and Health services to complete the referral process to request services.   Once the referral has been submitted, a member of the team will be in touch with you.
